Advanced Urgent Care Center in Dallas, Texas, is there to care for you when you have urgent and emergent medical needs that require consultation with our medical team such as Doctors and other Providers. Our team of health care staff is here to treat and care for most of your medical and health care issues including common cold, upper and lower respiratory infections (flu, strep throat, sinusitis, pneumonia, COPD etc.), gastrointestinal problems, dermatologic problems, hypertension, diabetes, urinary issues, lacerations, incision/drainage, fractures, and various other emergency medical/health issues.
The family medicine at Advanced Urgent Care Center is dedicated to prevention and control of diseases through immunizations/vaccinations, annual physical examinations, sports physical examinations, biometric screenings, free blood pressure checks, overall wellness examination.
The occupational branch of Advanced Urgent Care Center focuses on work related injuries including on the job falls, head injuries, lower, middle, and upper back injuries, neck injuries, upper and lower extremities injuries, hip/pelvic injuries, DOT physical examinations, pre-employment physical examination, DOT and non-DOT drug screens, collection of court ordered DNA/paternity tests, spirogram exam, physical therapy. Our Certified DOT medical examiners/Providers have extensive years of professional experience with work, and department of transportation related issues.
An added advantage of Advanced Urgent Care Center is its proximity (less than a minute) to the emergency room of Charlton Methodist Hospital in Dallas which makes necessity for higher care very easily accessible. Also, our hours and days of operations are designed to make us available to you to ensure you have access to medical care when you need it. We are committed to providing excellent health care services to you, our patient. As a part of our professional relationship, it is imperative that you have an understanding of our financial policy.
All patients must read and sign this form prior to receiving services.
It is your responsibility to provide us with your most current insurance information.

If you fail to provide accurate insurance information in a timely manner, your insurance company may deny the claim.

If the claim is denied, you will be financially responsible for services rendered.

We must emphasize that, as medical providers, our relationship is with you, the patient, and not your insurance company. Your insurance is a contract between you, your insurance company, and possibly your employer. It is your responsibility to know and understand the level of services covered by your insurance company.

We may accept assignment after verification of your coverage. Please be aware that some or perhaps all of the services provided may not be covered in full by your insurance company. You are financially responsible for services not covered by your insurance company.

Before receiving services, you must verify that we are participating providers for your insurance company. It is also necessary that our primary care physician is listed as your primary care provider with your insurance company, if required by your contract with your insurance company. In the event we are not participating providers or our physician is not listed as your primary care provider with your insurance company, we will fine the initial claim as a courtesy. Payment, however, is due in full at the time of service.

We charge what is usual and customary for our area. You are responsible for payment regardless of any insurance company’s arbitrary determination of usual and customary rates.

Co-payments, coinsurance and/or deductibles are due at the time of service. We will estimate the amount you owe based on information we receive from your insurance company. However, you are responsible for paying the full amount determined by your insurance company once they have paid your claim – regardless of our estimation.
